BENEFIT SUMMARY SHEET FOR EMPLOYEES
AFLAC
Several different plans available from the representative through payroll deduction. Also available to pre-tax health and dental insurance.
Auto. Payroll Check Deposit
To any bank, credit union, or savings and loan each pay period.
Breaks
20 minute paid break and 30 minute unpaid lunch per 8-hour shift
Call-Back Pay
Employees called back in to work receive time and one-half for any callback hours (one hour minimum).
Credit Union
Checking, savings, and low interest loans through the Deere Community Federal Credit Union.
Dry Cleaning
Free drop off and pick up service as needed. Call 642-3711 for service.
Dining Area
Food available 12:00 to 1:00 . Tickets available at the Information Desk.
Education
ACLS and PALS training offered to staff members.

Employee Assistance Program
Assistance Center at St. Luke's Hospital in Cedar Rapids - will provide free counseling sessions (up to 3) for employees and family members plus referral.
Flexible Spending Account
Unreimbursed Medical and Dependent Child Care. Pre-tax $ deducted for
medical/dental/vision expenses not covered by insurance or for child care expenses.
Free Parking
Parking located north of the hospital in the lot and east of the hospital on the far east end of the lot
Health/Dental Insurance
Hospital pays 80% of Blue Advantage Premium and 65% of other Advantage plan. Hospital pays 50% of Alliance Select Plan. Dental Insurance is offered, employee pays 100% of plan.
Holidays
Holidays- 5 paid holidays:
- Memorial Day
- July 4 th (Independence Day
- Labor Day
- Thanksgiving Day
- Christmas Day
IPERS
Hospital contributes 6.65% of employee's gross earnings and employee contributes 4.3% of their gross earnings toward Iowa Public Employees' Retirement Plan.
Life Insurance
Life Insurance is offered to eligible full time employees. Employee pays 100% of policy.
Malpractice Insurance
Liability insurance coverage while performing your assigned duties.
Shift Pay
Additional $.50/hour for 2 nd shift and weekends; and $1.00/hour for 3 rd shift
Short/Long Term Disability
Short term is offered to all eligible full time employees. Employee pays 100% of short term policy.
Long term monthly max of $5000.00. Long term policy is paid for by hospital.
PTO
Accrued on hours worked.
Wellness
Free use of equipment in Rehab area. Free flu shots when available.
